Abstract :Etiological diagnosis of pleural effusion is sometimes easy and sometimes very difficult challenge. In most of patients with pleural effusion, the symptoms, signs and laboratory data are not pathognomonic for etiologic diagnosis. In a patient with an undiagnosed pleural effusion, the first question to answer is whether the fluid is a transudate or an exudate. This is usually determined by means of Light`s criteria. In patients under diuretic treatment, Light`s criteria misclassify transudates as exudates, but the pleural fluid NT-pro-BNP levels usually is above 1500 pg/mL in pleural effusions associated with heart failure. Keywords : pleural effusion, new biomarkers
