Abstract :Choanal atresia is the most frequently detected congenital nasal anomaly seen in one of every 5 to 8 thousand births. The symptoms seen in unilateral choanal atresia are mild and might be overlooked until adulthood. The major complaints in unilateral choanal atresia are nasal stuffiness and rhinorrhea. On the other hand bilateral choanal atresia is a life threatening condition that causes airway problems, and so should be early diagnosed and treated. In our case report we represent a transnasal endoscopic approach on a 43 years old unilateral choanal atresia patient who was operated two times because of nasal stuffiness complaints
